For Candidates Born Outside Gujarat

• Domicile certificate issued by the authority empowered by the Government of Gujarat

Proof of Birth Date

• Birth certificate

• Indian passport

• Standard 10th (SSC) mark-sheet

For Scheduled Tribes (ST) candidate

• Certificate belonging to Scheduled Tribes (ST)issued by the authority empowered by the Government of Gujarat on that behalf

For Scheduled Caste (SC) candidate

• Certificate belonging to Scheduled Caste (SC) issued by the authority empowered by the Government of Gujarat on that behalf

For Economically Weaker Sections (EWS) candidate

• Certificate belonging to Economically Weaker Sections (EWS) issued by the authority empowered by the Government of Gujarat on that behalf on or after 01.04.2021

For Socially and Educationally Backward Classes (SEBC) candidate

• Certificates belonging to Socially and Educationally Backward Classes (SEBC) and

• Non-Creamy Layer (NCL) certificate of the family, as per the prevailing policy of the Government of Gujarat issued on or after 01.04.2021

• Note: Both certificates were issued by the authority empowered by the Government of Gujarat on that behalf

For a Person with Disability (PwD) candidate

• Certificate of Disability issued by the authority empowered by the Government

For the local candidates of Ahmedabad

• The certificate of the local candidate from the Dean of Smt. NHL Municipal Medical College, Ahmedabad (It will be issued only after the publication of NEET-UG-2025 results)

For the local candidates of Surat

• The certificate of the local candidate from the Dean of Surat Municipal Institute of Medical Education and Research, Surat(It will be issued only after publication of NEET-UG-2025 results)

Certificate Physical Fitness

• Certificate of Physical Fitness in the prescribed form obtained from a registered medical practitioner (M.B.B.S/MD/MS) along with the undertaking.

• Note: The prescribed form will be available on the official website of ACPUGMEC

For Overseas Citizens of India (OCI) candidate

• OCI card issued by the competent authority of Central Government of India (Only for NRI Quota admission)

Sons and daughters of defence personnel

• Domicile certificate of defense personnel issued by the authority empowered by the Government of Gujarat and Bonafide certificate issued by the competent authority

Sons and daughters of IAS/IPS/IRS personnel

• Details of Original Posting in Gujarat State & thereafter current transfer order

Students who passed 12th from Jawahar Navoday Vidhyalaya outside Gujarat State

• Passing mark sheet of Std.1st to Std.8th issued from School located in Gujarat State

Students who passed std. 12th from Gujarat Board of Union Territories of Daman & Diu and Dadra & Nagar Haveli

• Std 12th Marksheet of Gujarat Board & Domicile certificate issued by the authority empowered by the Government of Gujarat
