A part of the UP NHM 2022 application process is the uploading of necessary documents, such as ID proof, qualifications and certificates, along with photographs and signatures. Here is a list of essential documents candidates must keep ready when filling out the application form:

A valid personal email ID and mobile no. for OTP registration (all essential details, like registration no. and password shall be sent to the same registered ID and contact no.)

• Marksheet/passing certificate of High School as age proof

• Degree/Diploma in the related field of study

• A valid related Registration certificate from the respective Council

• Scanned recent passport-size colour photograph (in the prescribed size and format)

• Scanned image of candidate’s signature in blank ink pen (in the prescribed size and format)

Candidates are instructed to carefully go through the instructions mentioned in the NHM UP recruitment 2022 notification regarding the specifications of the photograph and signature before applying online. The last date for submission of the application form is December 12, 11:55 PM.
