• Scanned copy of Photograph of the candidate with date and name (5H x 4W cm size, 30 to 50 KB)
• Scanned copy of Signature of the candidate (2H x 4W cm size, 30 to 50 KB)
• Mobile Number (Active)
• Valid email ID
• Caste Certificate (if applicable)
• SAP, Specially Abled Person Certificate (if applicable) indicating 40% above disability
